Summary of case history ,The patient Wang, 66 years old, went to the doctor on September 1, 2020 due to "edema with foam in urine for 5 years, aggravated in March". 11 years ago, the patient's blood sugar was found to be elevated in physical examination and was not treated. 5 years ago, he developed edema of both lower limbs and foam in urine. He was hospitalized in the Army General Hospital and diagnosed as diabetic nephropathy of type 2 diabetes mellitus, which improved after treatment. After 3 months, the symptoms became worse, and he went to Dongzhimen Hospital of Beijing University of Traditional Chinese Medicine for treatment of hypoglycemia, hypotension and diuresis. Since then, the patient has intermittent edema of both lower limbs with more foam in urine. In order to further diagnosis and treatment, he went to our clinic. Symptoms and signs: the patient's face is pale yellow, body is fat, both lower limbs have severe concave edema, limbs are heavy, fatigue, more secretions from both eyes, can sleep well, there is foam in urine, and stool is sticky once a day. The tongue is light and slightly dark, the fur is thin and white and greasy and slightly yellow, and the pulse is heavy and smooth.,TCM diagnosis ,diabetes nephropathy (Qi deficiency and blood stasis, stagnation of dampness and turbidity, and wind evil).,Western medicine diagnosis ,Diabetes kidney disease.,Therapeutic methods ,The traditional Chinese medicine takes the methods of tonifying qi and activating blood circulation, promoting water and clearing heat, dispelling wind and removing turbidity as the methods, and gives the self-prepared traditional Chinese medicine orally. Western medicine gives symptomatic treatment such as lowering blood pressure, lowering blood sugar and anti-platelet aggregation.,Clinical outcomes ,Renal function was improved, albuminuria was significantly reduced, edema and discomfort symptoms disappeared.
