Summary of case history,　A case of secondary erythematous acrodynia complicated with gangrene and bone necrosis at the toe end is reported. The patient, male, 73 years old, was hospitalized in our outpatient department because of "erythema swelling and pain and burning sensation of both feet for 8 years, and then aggravated for 3 months, necrosis and ulceration for 1 month". The patient showed continuous burning like pain. The pain was intense, alleviated in cold and aggravated in heat, and could be slightly relieved by soaking his feet in cold water every day. Platelet examination after admission plt:492 × 10,9,/L; CTA of both lower limbs: multiple arteriosclerosis of large vessels of both lower limbs, including local severe stenosis of bilateral anterior tibial arteries and mild stenosis of the remaining segments. Dermatological examination: high edema of both feet and lower limbs, extensive edematous dark red and purplish red erythema, edematous purpura edematous dark spot at the distal end of the ten toes, gangrene. The consultation of vascular surgery and orthopaedics department in many hospitals suggests amputation treatment.
