Neck and shoulder pains, along with limited head and neck movement are symptoms commonly seen in cervical spondylosis. The X-ray findings of patients are often presented as the straightened cervical curvature, C3-C7 hypersteogeny and other pathological manifestations. These are often a series of compensatory results due to neck muscle tissue strains leading to the limited flexibility of the head and neck. As such, cervical spondylosis treatment should be centered on improving the flexibility of the head and neck as well as relieving muscle strain on the neck. With the "zhatiao" acupuncture technique to relax targeted muscles, acupuncture is able to relieve the abnormal tension of the muscles and restore its physiological function. Treatment includes the selection of posterior rectus major and inferior oblique muscles which can limit the flexibility of the atlantoaxial joint, as well as sternocleidomastoid muscle and splenius capitis muscle which can affect the left and right rotation of the head and neck as the target muscle groups. This paper discussed the basis behind local acupoint selection which aimed at restoring the flexibility of atlantoaxial joint as well as left and right rotation of the head and neck, the selection method used for treatment points, and "zhatiao" acupuncture technique, along with typical case reports in the treatment of cervical spondylosis.
