摘要

目的,2,观察二补助育汤对反复体外受精-胚胎移植（IVF-ET）失败患者子宫内膜超声学指标的影响，探讨该方药对子宫内膜容受性的改善作用。,方法,2,采用自身前后对照试验，纳入32例优胚移植2次及2次以上均失败、除外胚胎质量因素的反复IVF-ET失败患者，且中医辨证为肾虚血瘀。观察二补助育汤治疗前后患者子宫内膜超声学指标的变化情况，以及再次IVF-ET的妊娠结局。,结果,2,32例患者中1例失访、1例因未坚持治疗剔除，最终30例患者纳入统计分析。患者治疗后子宫内膜厚度较治疗前增加（,P,<,0.001），A型子宫内膜患者数量、子宫内膜下存在血流的患者数量均较治疗前增加（,P,<,0.001），Salle评分较治疗前明显增加（,P,<,0.001），双侧子宫动脉搏动指数（pulse index, PI）、阻力指数（resistance index, RI）和左侧子宫动脉收缩期峰值/舒张期峰值（S/D）较治疗前明显降低（,P,<,0.05,P,<,0.001）。再次行IVF的胚胎种植率和临床妊娠率分别为40%、33.33%。,结论,2,二补助育汤能增加子宫动脉血流，从子宫内膜厚度、分型、血供等方面改善子宫内膜容受性，利于胚胎着床，从而提高再次IVF-ET成功率。

Abstract

Objective,2,To observe the influence of ,Erbu Zhuyu Tang, (Double Tonifying Medicinals for Helping Pregnancy Decoction, EZT) on endometrial ultrasound indexes in patients with repeated failures of in vitro fertilization-embryo transfer (IVF-ET), and discuss the improvement effect of EZT on endometrial receptivity.,Methods,2,The patients (,n,=32) with repeated IVF-ET failures (2 times or more), except of factor of embryo quality, were chosen by using self-control test, and they were diagnosed as pattern of kidney deficiency and blood stasis with pattern differentiation of Chinese medicine. The changes of endometrial ultrasound indexes and pregnant outcomes after once again IVF were observed in the patients before and after treatment with EZT.,Results,2,Among 32 patients, one lost to follow-up and one was excluded because of non-adherence treatment, and f inally 30 patients were included into statistical analysis. The endometrial thickness increased(,P,<,0.001), and numbers of patients with type A endometrium and blood flow under the endometrium increased (,P,<,0.001) after treatment. Salle scorse increased significantly after treatment (,P,<, 0.001). The pulse index (PI) and resistance index (RI) of bilateral uterine artery, and S/D ratio of lef t uterine artery decreased significantly (,P,<,0.05,P,<,0.001) after treatment. The rate of again IVF-ET was 40% and clinical pregnancy rate was 33.33%.,Conclusion,2,EZT can improve blood flow of uterine artery, and endometrial receptivity in aspects of endometrial thickness, types and blood supply, which is conducive to improvement of embryo implantation and success rate of again IVF-ET.
