Summary of case history,　A 5-year-old male patient named Tang was admitted to the Department of Otolaryngology on May 30, 2022(Solar term: Grain Buds). The patient complained of bilateral aural fullness and hearing loss for more than 4 months, whose prodromal stage was characterized by cold symptoms. He was administrated with antibiotics, nasal spray and other Western medicines in the local hospital for more than 3 months, but to no avail. The surgeon suggested adenoidectomy, but the child's parents refused the operation. Then he received Chinese herbs, clearing away heat and resolving phlegm and suppressing cough for more than one months, little relief of the symptoms could be achieved. The patient had a history of nasal congestion and rhinorrhea in the past two years. Patient's concerns were manifested as a feeling of fullness in both ears, hearing loss, nasal obstruction, and watery nasal discharge. Other associated symptoms were loss of appetite, sloppy stool and snores. The patient's complexion was not enough rosy and the body was thin and small. Only did the speaker raise the voice it enable the patient to listen clearly. His tongue and pulse manifestation was pale red tongue with white-slimy fur and fine-slippery pulse. Physical examination showed that the bilateral tympanic membrane was orange-red with effusion sign and the nasal mucosa was light red and both inferior turbinates were swollen. Pure tone audiometry showed bilateral conductive deafness and acoustic immittance measurement showed the type B tympanic immittance pattern of the right ear and the type C tympanic immittance pattern of the left ear, and the stapedius reflex of both ears disappeared.,Western medicine diagnosis,　bilateral secretory otitis media was diagnosed.,TCM diagnosis,　The traditional Chinese medicine diagnosis of the patient was ear distension and the Pattern was spleen deficiency with dampness encumbrance,Therapeutic methods,　The corresponding treatment principles was fortifying the spleen, harmonizing the stomach, resolving dampness and inducing resuscitation. The doctor also suggested to go to bed early and rise early, advoid consuming rich, sweet and greasy food as well as cold and raw food, and keep warm.,Clinical outcomes,　After 4 weeks of treatment with pure Chinese medicine, a total of 3 times of treatment and 28 doses of Chinese medicine, the patient was cured and discharged from hospital with all symptoms disappeared. The tympanic membranes, pure tone audiometry and acoustic immittance examination were restored to normal.
