病史摘要,2,孙某,青年男性,2021年8月23日因“间断性腹泻34余年,加重5 d伴嗜酸性粒细胞增多”就诊,完善外周血细胞计数、白细胞分类、生化全项、胸部CT、骨髓形态、流式免疫分型、融合基因、FISH、染色体、骨髓病理、结肠镜及组织病理学等检查,西医诊断为高嗜酸粒细胞增多症,此病病因尚未彻底明确, 暂无特效治疗方案,一般以重要器官受累和功能障碍作为主要治疗指征,因该患者无靶器官损害,西医暂无特殊治疗方法。中医根据患者腹泻长年反复发作,便质稀薄,畏寒畏风,腹泻遇寒加重、食冷加重,腹痛得温痛减,四诊合参,中医诊断为泄泻之脾肾阳虚证,以温中散寒、升提中气、涩肠止泻为治疗原则,方选附子理中汤合补中益气汤加减,单纯用中药治疗2周后,患者腹泻明显缓解,定期复查血常规提示嗜酸性粒细胞逐渐下降,3个月后随访,遇寒遇风均未再出现腹泻,饮食同常人,疗效满意。
A young male,named Sun, who present spontaneous because of "intermittent diarrhea for more than 34 years, aggravated for 5 days,with eosinophilia". By a series of tests inlcuding peripheral blood cell count, leukocyte classification, biochemical items, chest CT, bone marrow morphology, flow immunotyping, fusion gene, FISH, chromosomal, bone marrow pathology, colonoscopy and histopathology, he was clearly diagnosed hypereosinophilia in Western medicine. Due to the cause of this disease has not been completely determined, there is no specific treatment plans, the main treatment indication is the involvements and dysfunction of important organs generally. At present, this patient has no target organ damage, so there is no special treatments for him in western medicine for the time being. According to symptoms that the patient has diarrhea repeatedly for many years, with thin stool, fearing of cold and wind, the diarrhea becomes worse when meeting cold wind or cold food, and the abdominal pain relieved by warm. With the combination of four diagnostic methods, he was diagnosed diarrhea belonging to the syndrome of Yang deficiency of spleen and kidney in TCM. Based on the principle of warming the middle energizer to dissipate cold, raising the Qi, and astringent bowel to stop diarrhea, we selected the Fuzi Lizhong Tang and Buzhong Yiqi Tang to treat this patient. After 2 weeks of treatment alone, the patient's diarrhea is significantly relieved. Regular blood routine examinations has showed that eosinophils decreased gradually. After 3 months of follow-up, his diet return to normal, no diarrhea occurred when get cold wind or cold food. The effect was satisfactory.
