Summary of case history ,The patient was admitted to the hospital due toparoxysmal joint redness, swelling and thermal pain for 10 years, tophi nodules for 4 years, and skin ulceration for 1 year. There was a 5-year history of hypertension. He likes to eat greasy food, tobacco and alcohol addiction, his father has a history of gouty arthritis. Admission symptoms: multiple joints of extremities redness, swelling, thermal pain; tophi of different sizes are seen in the limbs, auricles, waist and abdomen; the skin of the outer legs and the front of the tibia is broken; white tophi can be seen under the skin around the wound. Diet is normal, sleep is not good, urine is little and red, Normal diet, poor sleep, little and red urine, red tongue,Yellow fur, and thready and slippery pulse. Auxiliary examination: creatinine 131.0 μMol/L, uric acid 589 mmol /L, erythrocyte sedimentation rate 36 mm/h, C-reactive protein 2.80mg/dl, abdominal color ultrasound showed bilateral renal stones.,TCM diagnosis ,Zhuo yu bi With dampness-heat amassment pattern,Western medicine diagnosis ,Gouty arthritis; Gouty nephropathy; Kidney stones; Hypertension,Therapeutic methods ,Miao medicine ,Tongfengting, decoction was given for internal administration, debridement of the local skin ulceration, and then atomized with Miao medicine Wuteng Powder. At the same time, tophi of both knees were cleaned with acupotomoscope. The Febuxostat tablets were used to reduce uric acid, and Prednisone acetate to relieve pain. After discharged, the patient continued to take Miao medicine Tongfengting decoction and Febuxostat tablets to reduce uric acid, and went to the outpatient department for wound dressing change every other day.,Clinical outcomes ,One month later, the patient's condition was stable and the skin ulcer healed basically. the patient was orderd to continue to take ,Tongfengting, decoction. During the follow-up for half a year, the patient maintained febuxostat every day, the blood creatinine and uric acid were normal,and some tophi nodules began to become smaller.
