- 摘要：Dry eye disease is a common ocular surface disease in Ophthalmology, which is characterized by the imbalance of tear film homeostasis.Patients with dry eye disease may have symptoms such as foreign body sensation, dryness and burning sensation.With the popularization of electronic products, the incidence of dry eye disease is increasing year by year, which can affect eyesight and people's normal life in severe times.At present, the treatment of dry eye disease is mainly artificial tears replacement therapy, which can only improve local symptoms, but can not be used for a long time. Dry eye disease belongs to the category of "white astringent disease" in traditional Chinese medicine.Traditional Chinese medicine has a long history in the treatment of dry eye disease, with good curative effect.Modern doctors have different views on the etiology and pathogenesis of dry eye disease, and the treatment is also different.This paper introduced professor XIE Like's clinical experience in the treatment of dry eye disease with the method of soothing liver and nourishing Yin for many years.On the basis of predecessors'knowledge and more than 30 years'clinical experience, Professor XIE Like thinks that the etiology and pathogenesis of dry eye disease are mostly stagnation of Qi, long-term change of fire, deficiency of body fluid, which are closely related to the liver, and puts forward "depression" and "deficiency" as the main pathological factors of the disease, summarizes the pathogenesis of dry eye disease as "liver depression and Yin deficiency", and thinks that the occurrence of the disease is similar to the stress reaction of the body.In clinic, Xiaoshengsan(逍生散) is used to treat dry eye disease, which plays the role of soothing liver and nourishing Yin.In addition, we should pay attention to the use of wind medicine, often combined with Fangfeng(Radix Saposhnikoviae), Juhua(Flos Chrysanthemi) and Sangye(Folium Mori). Its main pathological features are ovarian enlargement, systemic capillary hyperplasia and permeability fluid exudation, blood concentration, and imbalance of water and electrolyte balance, often seen in patients with polycystic ovary syndrome. "One Qi" here refers to zhong Qi (middle Qi), and "four images" refer to wood, metal, fire and water organs(i.e., the liver, lungs, heart and kidneys). According to the "One Qi Circumfluence" theory, under the effects of zhong Qi, the liver on the left ascends and the lungs on the right descend, forming an "endless circulation" of the Qi activities in the human body. Vocal cord leukoplakia is a white plaque formed by keratinized hyperplasia or excessive keratinization of vocal cord mucosal epithelium. It belongs to the category of "slow throat silence" in traditional Chinese medicine. The abnormal rise and fall of Qi runs through its pathogenesis. The abnormal mediation of Qi is the source of the disease, and the dereliction of the liver and lung Qi activities is an important cause. In the treatment of vocal cord leukoplakia, it is emphasized to restore the rise and fall of Qi activities. Traditional Chinese medicine has unique advantages in treating arteriosclerosing occlusive disease. The theory of blood-vessel syndrome differentiation takes vessels as the main body and basic framework, and blood as the material basis. It holds that abnormal shape and dysfunction of vessels are two key factors in the pathogenesis of peripheral vascular diseases.The physiological characteristics of vessels are discussed by taking vessels as the physique and dredging for use, and the function of vessels is discussed by obstructing blood to communicate with ying wei and carrying Qi and blood to carry body fluids. Physiologically, blood favors warmth and dislikes cold and it serves "to help and nourish the physique and spirit" . The disease location of arteriosclerotic occlusive disease is in blood, with the blood stasis of limbs as its core pathogenesis. Based on the theory of blood-vessel syndrome differentiation, arteriosclerotic occlusive disease is divided into the patterns of yin and cold of the collaterals, blood stasis of collaterals, blood stasis with heat in the collaterals, and blood stasis and toxin accumulation in the collaterals. Firstly, this paper introduces the theoretical origin of treating headache by dividing meridians in past dynasties, and points out that there are defects in the principle of selecting acupuncture points in the chapter of headache in acupuncture textbooks by analyzing the theory of meridian circulation, and further discusses that the application of dividing meridians in treating headache in acupuncture should be different from the application of medicine returning to meridians, which should not only consider the application value of special medicine returning to meridians in selecting acupuncture points according to meridians, but also recognize the complexity of meridian system in different parts of the head. Zhou Zheng-hua has found that there aremany subtypesof liver-stomach disharmony, and the pathogenesis follows the five-step evolution processof＂depression→attacking thoroughfare vessel (TV) →transforming into fire→damaging yin→forming blood stasis＂. In the initial stage, the patho-genesis is stagnation of liver qi, wood failing to disperse earth. Adhering to the principle of controlling developmentof existing dis-ease, because＂liver diseasesmay affect the function of the spleen, strengthening the spleen is themost important＂. At this stage, Four Gentlemen Decoction can be used to strengthen the spleen and help transport. When the stagnation of liver qi develops fur-ther, the disorder of liver qi occurs, the dispersing is toomuch, the attack of liver qi is too strong, the treatment should focus on reg-ulating qi, so Frigid Extremities Powder combined with Modified Upper-Lower and Right-Left Decoction can be used. Long-time stagnation of liver qi may transform into fire, make qi and fire have same disease, so Liver-Resolving Decoction combined with Modified Gallbladder-Warming Decoction can be used. For liver heat and spleen cold syndrome, Bupleurum and Cinnamon Twig and Dried Ginger Decoction can be used towarm cold inmiddle energizer and clear heat in the revertingmeridian. If liver qi accu-mulates and transforms into fire, damaging liver qi aspect, Yang hyperactivity and excessive fire will burn blood aspect, blood will become thick and fail tomove smoothly, resulting in blocked collaterals, blood stasis, and fire damaging nutrient yin, so the treat-ment is to soften the liver, benefit the stomach, nourish yin and calm themind, and Yin-Nourishing and Two-Ingredient Decoction combined with Modified Jade Lady Decoction can be used. For patientswith FD who have nothealed for a long time but combined with obvious blood stasis, the treatmentmethods are also differentiated according to blood stasis caused by different pathogenesis. Each stage can exist independently or cause disease simultaneously.
