摘要

慢性肾脏病（CKD）已成为全球性公共卫生问题，肾脏纤维化（RF）作为CKD终末阶段的共同病理基础，早期干预对延缓CKD进展具有重要意义。曹式丽教授认为RF病机乃风、湿、瘀毒胶痼少阴肾络，故仿张仲景《金匮要略》治疗疟母的鳖甲煎丸之法，结合吴鞠通《温病条辨》青蒿鳖甲汤条文的“先入后出”之论，提出“从阴引阳”治则并形成特色中医临床思维。临证仿张仲景虫药用法，以动态之虫蚁领药搜剔肾络，辅驱邪补益之品，疏络逐邪，从阴达阳，给邪出路，以使风毒自少阳转枢而达表、瘀毒借阳明谷道而下行、湿毒从少阳三焦而分消。本文从治则治法角度探讨曹式丽教授治疗肾脏纤维化的临床思维，以期为开展其诊疗肾病的学术思想提供理论依据。

Abstract

Chronic kidney disease (CKD) is now a global public health problem. Renal fibrosis (RF) is a common pathological basis in the end stage of CKD. Early intervention of RF has important clinical significance and scientific research value in delaying the progression of CKD . The famous Traditional Chinese Medicine (TCM) professor, CAO Shili, thought that the pathogenesis of RF is toxin of wind, dampness, and blood stasis in the Kidney Collateral of Foot-Shaoyin. She imitated the method of Turtle Shell Decoction of the Golden Chamber and combined with the theory of "first in and then out" of Sweet Wormwood and Turtle Shell Decoction in Wu Jutong's Detailed Analysis of Epidemic Warm Diseases. Then She proposed the rule of "from Yin to Yang" and formed a characteristic Chinese medicine clinical thinking to prevent and treat RF. First, the anti pathogenic and tonic drugs are led into renal meridians by insect drugs, and then put the evil to Yang. The invisible evil has turned out from the Shaoyang. The blood has stabbed by Yang Ming. Dampness is removed from Shaoyang Sanjiao. It can delay the progress of ESRD, and have significant curative effects. Her distinctive clinical thinking has been formed. We have sorted out this characteristic clinical thinking, and provided the basis for the subsequent academic thought and experimental theoretical basis of CAO Shili's TCM nephropathy.
