CAO Jianchun's Treatment of leg Cramp with the Combination of Acupuncture and Medicine
病史摘要,2,主诉:右下肢抽筋20年,加重2年。现病史:患者20年前无明显诱因出现右下肢抽筋,夜间发作频繁,伴有双下肢沉重、乏力。自行服用钙尔奇后,自觉症状好转。6年前外院诊断为“双下肢静脉功能不全”。近2年自觉症状加重,右下肢抽筋5~6次/日,夜间尤甚,服用钙尔奇后无明显缓解。刻下症见:双下肢静脉迂曲扩张,局部成团块状,右侧为甚;双下肢轻度可凹性水肿,活动、劳累后沉胀感加重;口干口苦,纳可,眠差,小便可,大便干,2日一行;舌淡黯,苔薄微黄,脉沉细。既往高血压病史10年,最高达150/110 mmHg;2型糖尿病病史10年,现降糖方案:二甲双胍0.5 g bid,阿卡波糖片50 mg tid。中医诊断 瘈疭,阴血亏虚证;水肿,湿瘀阻络证西医诊断 右下肢肌肉痉挛,下肢静脉功能不全,下肢静脉曲张,高血压1级(中危),2型糖尿病,糖尿病性周围神经病变干预措施 口服中药颗粒剂,配合针刺疗法,降压药物暂未予,降糖药物续前方案,穿戴压力梯度弹力袜,休息时抬高患肢,适当运动,规律饮食。疗效转归 经中药内服和针刺治疗后,患者小腿抽筋症状明显好转,偶发一月1~2次,睡眠质量随之大幅提高;双下肢仍有静脉曲张表现,但沉重、乏力症状减轻,水肿改善。随访得知患者现规律穿戴压力梯度弹力袜,血压、血糖控制稳定;现已停服中药,配合门诊针刺治疗巩固,延缓疾病进一步发展。
Abstract
Summary of case history, Main complaint: right lower limb cramp for 20 years, aggravated for 2 years. Current medical history: 20 years ago, the patient had no obvious inducement to have cramps in the right lower limb, and had frequent attacks at night, accompanied by heavy and fatigue of both lower limbs. After taking "Caltrate" by herself, the symptoms were improved. Six years ago, the hospital diagnosed "venous insufficiency of both lower limbs". In the past 2 years, the symptoms have been aggravated, and the right lower limb cramps have been 5~6 times / day, especially at night. There is no obvious relief after taking "Caltrate". The following symptoms are seen: the veins of both lower extremities are tortuous and dilated, with local clumps, especially on the right side; Mild concave edema of both lower extremities, aggravation of heaviness after activity and fatigue; Dry mouth, bitter mouth, Normal appetite, poor sleep, urination, dry stool; The tongue is pale, the moss is thin and yellowish, and the pulse is thin. The previous history of hypertension was 10 years, up to 150/110 mmhg; The history of type 2 diabetes is 10 years, and the current hypoglycemic regimen: metformin 0.5g bid, Acarbose Tablets 50mg tid.,TCM diagnosis, convulsions,syndrome of deficiency of yin and blood; edema, dampness and blood stasis blocking collaterals syndrome.,Western medicine diagnosis, muscle spasm of right lower limb, venous insufficiency of lower limb, varicose veins of lower limb, hypertension grade 1 (moderate risk), type 2 diabetes, diabetes peripheral neuropathy.,Therapeutic methods, Oral Chinese medicine granules, combined with acupuncture therapy. antihypertensive drugs are not given for the time being, and hypoglycemic drugs are continued. Wear pressure gradient elastic socks, lift the affected limb during rest. exercise properly, and eat regularly.,Clinical outcomes, After oral administration of traditional Chinese medicine and acupuncture treatment, the patient's leg cramp symptoms were significantly improved, occasionally 1~2 times a month, and the sleep quality was greatly improved; Varicose veins were still present in both lower limbs, but the symptoms of heaviness and fatigue were reduced, and edema was improved. The follow-up showed that the patient was wearing pressure gradient elastic socks regularly, and the blood pressure and blood sugar were stable; It has stopped taking traditional Chinese medicine and cooperated with outpatient acupuncture treatment to consolidate and delay the further development of the disease.
