Treatment of Severe Pneumonia Complicated with Encapsulated Pleural Effusion with Dachaihu Decoction: A Case Report
Summary of case history, Patient Li Mou, female, 40 years old. On 31-May-2021, he visited the doctor due to "dyspnea with left chest pain for 1 week". One week ago, the patient experienced dyspnea accompanied by left chest pain with tingling pain, aggravated during breathing, accompanied by chest distress and palpitation after exercise. The patient did not pay attention to and received treatment, and the symptoms of dyspnea and chest pain were progressively aggravated, intermittent cough and expectoration with sticky sputum were difficult to cough. The patient came to our outpatient department for further diagnosis and treatment. Symptoms and signs: The patient had pale complexion, obesity and mental lassitude. Low voice, shortness of breath, dyspnea with chest distress and palpitation after activity, aggravated left chest pain during breathing, no wheezing sound, cough and expectoration, little sputum, white sticky sputum difficult to cough, sweating when moving, no aversion to cold and fever. Poor appetite, poor sleep at night, cold abdomen, loose stool, 1-2 times/day, dark yellow urine. Red tongue, thin yellow fur, floating and rapid pulse.,TCM diagnosis, Xuanyin (Taiyang syndrome).,Western medicine diagnosis, Community-acquired pneumonia; Left pleural effusion; Pericardial effusion; fatty liver.,Therapeutic methods, Traditional Chinese medicine ( TCM ) is based on syndrome differentiation of six meridians. At the initial diagnosis, it is treated with Guizhi Renshen Decoction by opening the sun, and then with Dachaihu Decoction by harmonizing Shaoyang and descending Yangming. Western medicine to anti infection, improve hypoxia, cough and phlegm treatment.,Clinical outcomes, The patient's chest effusion subsided significantly and the inflammation was significantly reduced. The outpatient follow-up patients were stable.
