A Case Report: Pro. OUYANG Huiqing Treats Pain Related to Endometriosis Based On Syndrome Differentiation of Viscera Under Holism
Summary of case history, A 39 year old female patient went to hospital because of "recurrent abdominal pain for 5 years after the second operation of endometriosis, headache and sleeplessness for 2 months". The patient had regular menstruation and obvious dysmenorrhea which Fenbid was needed during period. Since the second operation, the patient had repeatedly experienced emotion related lower abdominal pain which had no fixed place and pain during ovulation period as well. Menstruation decreased gradually. She was diagnosed depression and related drug was recommended. She refused western medicine treatment, therefore, looked for the method in Chinese medicine. Gynecological examination showed that painful nodules were palpable in the uterine rectal recess, the normal size uterus was plump, posterior position, fixed and tenderness pain. The bilateral tubo-ovarian were obviously thickened and tender. The fixed cystic mass was palpable in the right rear of the uterus, without obvious tenderness. The tongue is dark red with thick yellow coating. The pulse is deep and thin. In 2009, she underwent laparoscopic surgery for ovarian endometrioma. She had a history of infertility for 10 years. From 2010 to 2013,in vitro, fertilization (IVF) failed twice. Form now on, she had only one pregnancy ended with spontaneous abortion. Ultrasound showed that cystic mass of right accessory were existed, considering the possibility of encapsulated effusion.,TCM diagnosis, Depression, abdominal pain, dysmenorrhea, infertility with stasis of spleen and liver, dystrophy of heart and kidney.,Western medicine diagnosis, Depression, endometriosis (stage Ⅳ), pelvic inflammatory disease, infertility.,Therapeutic methods, The patient refused to be treated with western medicine. The therapy in TCM is regulating the liver and spleen, nourishing the heart and benefiting the kidney. Professor OUYANG Huiqing prescribed modified Yuejuwan.,Clinical outcomes, After treatment, the quality of sleeping, the mood and headache were significantly improved. The sweating was reduced. The abdominal pain and dysmenorrhea were significantly relieved, and no painkillers were needed. The menstrual volume was increased.
