摘要

病史摘要,2,患者白某，男性，59岁，2021年8月20入院，主诉：间断右上腹疼痛不适1月余，加重伴发热1周。既往体健，否认糖尿病、高血压、恶性肿瘤病史，否认胆囊结石病史，否认胆道及胰腺手术史。查体：T 38.8 ℃，HR 90 bpm，RR 20 bpm，BP 100/70 mmHg，SPO2 98%（未吸氧），神清，精神可，急性病容，皮肤巩膜无黄染，心肺未见异常体征，腹膨隆，未见胃肠型及蠕动波，无腹壁静脉曲张，全腹无明显压痛，无肌紧张及反跳痛，Murphy征（-），肝区叩击痛（+），移动性浊音阴性，肠鸣音3次/分。中医四诊：望：神色羸弱，面色萎黄，神志清楚，舌红紫起芒刺，苔黄厚腻。闻：气味如常，语速缓慢。问：右胁不适，食欲不佳，小便短赤，大便秘结。切：左脉悬浮，右脉滑数。西医诊断：细菌性肝脓肿；中医诊断：肝痈，热毒瘀肝。西药治疗：超声导向下肝脓肿穿刺引流，比阿培南联合替加环素抗感染，谷胱甘肽保肝，静脉营养支持，布氟比洛芬脂对症止痛；中医治则：清热解毒、活血化瘀、托里排脓、通里攻下，处方：五味消毒饮合膈下逐瘀汤加减；中医非药物治疗：穴位按摩、耳穴压豆。患者经中西医结合系统治疗后保留肝脓肿穿刺引流管、好转出院，继续门诊辩证施治、随证加减治疗1月后拔除肝脓肿穿刺引流管，患者痊愈。
