A Case of Children's POEMS Associated Castleman Disease in Convalescence Treated Mainly by Acupuncture增强出版
Summary of case history, Mr. CHEN, 14 years old. The main reason was that the patient had intermittent fever, diarrhea and vomiting in August 2020, followed by fatigue of both lower limbs. He could walk independently when he was in fashion. After symptomatic treatment such as anti-inflammatory treatment, fatigue of both lower limbs had not been relieved, and progressive aggravation, accompanied by intermittent diarrhea and vomiting. By October 2020, he had been unable to walk and appeared fatigue of both upper limbs, so he was hospitalized in Beijing Children's Hospital. After a series of examinations, he was diagnosed as polyneuropathy-organomegaly-endocrinopathy-monoclonal protein-skin changes (POEMS) associated Castleman disease, renal insufficiency, liver function damage, etc. After symptomatic treatment with lenalidomide, tozumab, glucocorticoid and other symptomatic treatment, his vital signs were stable, and he still left limb weakness and sensory allergy in both lower limbs. He came for traditional Chinese medicine(TCM) treatment. TCM syndrome: weakness of limbs, poor distal strength, uncoordinated movement, sore feet, blurred vision, back pain, banded pigmentation with skin lesions can be seen all over the body, irregular stool, once a day. The four diagnostic information Mind is clear, and the body is fat. The voice is clear, the breath is symmetrical, and there is no abnormal smell. The tongue is light and fat, the moss is white and greasy, and the pulse is thin.,TCM diagnosis ,Paralysis disease. Weakness of spleen and stomach.,Western medicine diagnosis ,POEMS associated Castleman disease.,Therapeutic methods ,Traditional Chinese medicine acupuncture takes invigorating the spleen and stomach, regulating and tonifying the five internal organs, relaxing tendons and activating blood as the treatment principles. Acupoint selection: Group 1: Zhongwan (CVl2), Guanyuan (CV4), Tianshu (ST25), Daheng (SPl5), shousanli (LI 10), Waiguan (TE5), Hegu (LI 4), Yuji (LU10), Taiyuan (LU9), Zusanli (ST36), Yanglingquan (GB34), Sanyinjiao (SP6), Taichong (LR3), Dadu (SP2), Taibai (SP3), rangu (KI2), Taixi (KI3). Group 2: Feishu (BL13), Geshu (BL17), Ganshu (BL18), Pishu (BL20), Weishu (BL22), Shenshu (BL23), Weizhong (BL40). Except Zhongwan (CV12) and Guanyuan (CV4), other acupoints were selected bilaterally. The two groups of acupoints were used alternately. Moxibustion points: Shenque (CV8). The lower limbs were washed with Notoginseng Radix et Rhizoma powder, Carthami Flos, Spatholobi Caulis, Persicae Semen, Angelicae Sinensis Radix, Aconiti Kusnezoffii Radix Cocta, Chuanxiong Rhizoma, Liquidambaris Fructus, Raw Rubiae Radix Et Rhizoma, Paeoniae Radix Rubra, Rehmanniae Radix, which can promote blood circulation and relieve pain. The children received the above treatments and got good results.,Clinical outcomes ,The curative effect evaluation is remarkable. Turn for the better.
