摘要

药物相关性颌骨坏死是双膦酸盐类药物潜在的严重不良反应，以坏死骨暴露、软组织脓肿至出现瘘管和弥漫性疼痛为主要临床表现。双膦酸盐类药物作为恶性肿瘤骨转移一线治疗用药，近年由其引发的药物相关颌骨坏死发生率有所升高，而临床缺乏有效治疗手段，值得引起关注。根据本病临床表现，与中医古籍中“骨槽风”相似，目前尚缺少中医药论治该病的临床报道。介绍1例乳腺癌骨转移患者应用唑来膦酸后导致下颌骨坏死，经外科手术及抗感染等治疗后复发加重的病例，通过中医辨病辨证相结合，考虑为正气无力托毒外出，导致败骨内留，局部毒热瘀邪相搏结，治以托里透脓、清热解毒、活血止痛，处方以仙方活命饮为主加减化裁，经5个月的治疗使患者局部症状消失，坏死骨脱落，破溃处愈合，取得了良好疗效。希望借此病例为药物相关性颌骨坏死的治疗提供思路。
